Athletic Physical Opportunities for Grades 6-12


We have several opportunities for student-athletes to obtain their required athletic physicals this summer.  All required paperwork must be on file with the Coach / Athletic Director before students can begin practicing. 

HealthPoint Family Care at Ludlow Schools:  On three dates this summer, the HealthPoint Office (housed in our school) will be open for appointments for athletic physicals (as well as other immunizations or visits).  The offices will be open on June 16, July 14, and August 11.  Contact Nurse Ashley Baumgardner to book your appointment.  You must have your HealthPoint packet completed if you are not currently registered with the office.

Forms to Be Submitted for Athletics at LHS:  
KHSAA Physical Form for Grades 6-8  or  KHSAA Physical Form for Grades 9-12
